How to refer

Send a referral the way that suits you. We accept Medicare CDM/EPC, DVA and NDIS referrals, and welcome privately funded and self-referred patients — referral templates available on request.
Medicare CDM / EPC

Team Care Arrangement or Chronic Disease Management referrals for eligible patients, with bulk-billing options. Send the referral and care plan and we'll take it from there.

DVA-referred and NDIS participants welcome — experienced across plan-managed, self-managed and agency-managed funding, building function and independence.
